Dr. Chappidi's office is conveniently located in the Professional Building of our Lady of the Resurrection Medical Center at the corner of Central and Addison on Chicago's northwest side. The address is 5600 West Addison, Suite 206. This site features free parking in the hospital's parking lot on Addison in addition to easy access by public transit.
